The membership interest form asks for your name and email address. We do not ask for or intentionally collect medical information, diagnoses, medication information, medical records, insurance numbers, Social Security numbers, banking details, credit-card details or financial-account information through these forms.
How we use information
We use submitted information to provide access to the demonstration, respond to membership interest, share caregiver resources and educational events, provide platform updates, and communicate about related services.
